Comagine Health is looking for an Improvement Advisor to enhance our work in Population Health Management Initiatives. This role will provide consultation and coaching, education and training, quality improvement and other practice transformation facilitation in healthcare facilities in collaboration with state departments of health, community partners, healthcare providers and other community partners. We consult on public health initiatives, designing and implementing healthcare delivery improvement projects across a variety of practice areas and populations.

Who is Comagine Health?
Comagine Health is a non-profit consulting firm that seeks to improve health and to increase the effectiveness and quality of health care. As a recognized Quality Improvement Organization (QIO), we support providers, plans, purchasers, and consumers, and offer services to state and federal agencies and others to help them better manage health care under the existing system and to assess, plan for and implement broader system transformation. We collaborate with academic, government, and nonprofit partners on initiatives funded by NIH, CDC, AHRQ, BJA, SAMHSA, and others.

Preferred applicants will have experience with Quality Improvement program development and implementation, population health initiatives in an outpatient care environment, and collaborating with diverse partners to achieve a common aim. You must reside in ID, NM, NV, OR, UT, or WA to be considered.
Required Skills*What would make you a strong fit for our role: *
Experience engaging outpatient care providers in quality improvement initiatives (preferred)
Excellent understanding of quality improvement concepts, tools, and techniques and how to apply them as an independent coach or part of a team
Intermediate proficiency with Microsoft Office Suite (Outlook, Word, PowerPoint, Excel)
Familiarity and comfort with videoconference technology (e.g., Zoom)
Ability to organize and coordinate multiple simultaneous tasks in a team environment
Excellent oral and written communication skills
Theoretical and practical knowledge of quality improvement methods and tools
Familiarity with health care terminology
Familiarity with health care clinical and operational workflows
Proficiency with project management concepts (e.g., project plans)
Hands-on experience with Customer Relationship Management (CRM) software and/or Microsoft Teams (preferred)
What you’ll be doing for us:
Consultation and Delivery of Technical Assistance
Collaborate with internal team members as well as external partners and other interested parties to improve health care delivery systems, processes, and outcomes efficiently and effectively
Provide coaching on data collection, interpretation, and utilization to drive performance improvement
Design and implement quality improvement interventions based on evidence-based and best practices
Project Management
Track project activities, including executing assigned tasks and/or phases of a project plan; monitor deployment of interventions to ensure program effectiveness, and report on progress to team lead
Training and Subject Matter Expertise
Design and implement learning collaboratives, learning and action networks, and other broad-scale delivery of quality improvement education and training
Provider and Community Partner Engagement and Convening
Conduct recruitment for QI initiatives, including outreach to providers and other organizations, creating, and managing recruitment plans and using data to target recruitment efforts
Convene multiple interdisciplinary partners to build consensus, shared vision, and action plans that contribute to the achievement of bold aims of health care and health improvement and population health initiatives
Required Experience
BA/BS in a health care, public health, or related field (Equivalent combination of education and/or work experience in related field may be substituted)
MA/MS in public health, nursing, health care administration, public policy, or any other related field (Equivalent combination of education and/or work experience in related field may be substituted)
4+ years of experience in health care, including quality improvement in health care settings
2+ years of project management experience
Comprehensive understanding of health care reform initiatives/environment.
