Job Description :
General Statement: The Alaska Primary Care Association (APCA) is a private, 501(c)(3) membership organization of Alaska’s “safety-net” primary care providers. The APCA’s mission is “helping to create healthy communities by supporting vibrant and effective community health centers” in Alaska. Our membership includes federally designated §330 Community Health Centers (CHCs) that serve all Alaskans and focus particularly on providing primary care services to those who are underinsured, uninsured, and live in medically underserved areas.
Position Summary: This position performs under the direction of the CHC Optimization Manager. The TTA Specialist is responsible for assisting with the project management of the Colorectal Cancer Control Program (CRCCP) grant including the training, support and mentoring of Patient Navigators and support of Colorectal Cancer Screening (CRCS) Provider Champions. This position will assist the project manager in tracking the various TTA project workplans and evaluations, meeting management, surveys, and website management.

Essential Duties and Responsibilities include the following. Other duties may be assigned as necessary.
Work as a member of the APCA Operations and Performance Excellence Services (Ops)
Assist with coordination of the Colorectal Cancer Screening Project
Assist with identifying patient navigators at each partner clinic
Assist with training Patient Navigators at each partner clinic to facilitate screening and reduce barriers to colonoscopy
Set up a peer network to support Patient Navigators
Assist Patient Navigators to support delivery of Evidence-Based Interventions (EBIs) and address structural barriers to implementation.
Monitoring and supporting implementation of EBIs in each clinic.
Assist with the annual review, revision and update of P&Ps for using limited funds to pay for follow-up colonoscopies for uninsured or underinsured adults age 50-75
Disseminate P&Ps to partner clinics
Track and report in aggregate for colonoscopies paid for with CDC funds
Assist with developing and implementing methods for securing treatment resources for patients diagnosed with cancer or needing additional evaluation or treatment
Assist with continued training of Patient Navigators to facilitate linkages to follow-up colonoscopies
Assist with continued training of Patient Navigators to facilitate screening and reduce barriers to colonoscopy completion regardless of the source of payment.
Communicate with participating health centers as necessary regarding the CRCS project.
Monitor the various (OPS) tracking tools for accurate completion by team members.
Assist departmental teams with project reporting requirements.
Assist with providing CHCs with a high level of customer service and support.
Organize and post to APCAs website, a library of useful PCMH and other QI data, reports, resources, and documents to serve as a helpful resource for CHCs to share/disseminate.
Develop effective relationships and ensure communication with partners and other key stakeholders.
Support department-wide communications efforts.
Other duties as assigned.
Qualifications / Knowledge / Skills and Abilities
Experience in health, social service, operations, process improvement or public administration fields desired. Community Health Center experience preferred.
Demonstrated knowledge, skill, and experience in working with diverse populations.
Excellent interpersonal, written, and oral communication skills supported by documented experience and professional references.
Demonstrated proficiency in use of a computer for word processing, spreadsheets, communication via email, and for conducting research via the Internet. Proficiency with Microsoft Office and Windows required. Must be willing to learn and use Office 365.
Ability to work effectively with a variety of professionals and organizations.
Ability to develop, maintain and promote constructive relationships.
Ability to work independently and to execute the direction of the CHC Optimization Manager in a timely, responsive, accurate and thorough manner.
Ability to execute discretion and independent judgment and maintain neutrality.
Supervisory Relationship: Received: Works under the direct supervision of the CHC Optimization Manager Supervision is received through review of work and observation of work in progress.Exercised: None
Language Skills: Fluency in written and spoken English required.
Travel Required: In-state travel required. Some out-of-state travel may also be required. Mileage is reimbursed per APCA policy.
Hiring Range: DOE/DOQ
Hours per Week: This is a full-time position; 40 hours/week.
